• Key Responsibilities:
• Deliver patient-centered care in a home health setting, in accordance with physician orders and agency policies.
• Conduct comprehensive assessments, develop personalized care plans, and coordinate with other healthcare professionals to ensure seamless care delivery.
• Empower patients and their families to take an active role in their care, promoting independence and self-management.
• Maintain accurate and timely documentation, adhering to regulatory requirements and agency standards.Requirements:
  • CURRENT RN licensure in the state of practice.
  • CURRENT CPR certification.
  • CURRENT Driver's License, vehicle insurance, and access to a reliable vehicle or public transportation.
